These Terms & Conditions apply to your use of sudburymedspa.com (the “Site”), including our contact and consultation request forms, operated by Sudbury Med Spa, 435 Boston Post Rd #6, Sudbury, MA 01776 (“we,” “us,” or “our”). By using the Site or submitting a form, you agree to these terms.
1. Information on this Site
Content on the Site is for general information only and is not medical advice. Treatment suitability, results, and pricing vary by person and can only be confirmed during an in-person consultation with our licensed providers. Submitting a form does not create a provider–patient relationship or book an appointment until we confirm it with you.
2. Using our contact form
Our contact form is provided only for current and prospective clients to ask about our treatments, request a consultation, or reach our team about their care. Please do not include detailed medical history in the form; we will collect that securely at your visit. You agree to provide accurate contact information and to only submit requests on your own behalf or with permission.
3. No solicitations — $1,000 fee
The contact form may not be used to send advertising, sales pitches, marketing, SEO, web design, lead-generation, or other commercial solicitations to Sudbury Med Spa. Anyone who submits the form for any of these purposes agrees to pay Sudbury Med Spa a fee of $1,000 (one thousand U.S. dollars) for each such submission, to cover the cost of our staff’s time in reviewing it, payable within 30 days of our invoice.
4. Communications
By submitting a form, you agree that we may contact you by phone or email about your inquiry. You can ask us to stop contacting you at any time by calling 978-579-4090.
5. Privacy
How we collect and use your information is described in our Privacy Policy and HIPAA Privacy Policy.
6. Bookings, gift cards, and purchases
Online appointment booking, gift cards, and product purchases may be handled by third-party services and are subject to their terms in addition to ours. Prices and promotions shown on the Site may change without notice.
7. Limitation of liability
The Site is provided “as is.” To the fullest extent permitted by law, Sudbury Med Spa is not liable for any indirect or consequential damages arising from your use of the Site.
8. Changes and governing law
